These resources are for students and faculty who are currently in our Nursing Program.
FORMS
Please submit these forms via email at gccnursing@glendale.edu.
- Department Petition | This petition should be filled out for any department requests such as schedule changes, clinical placements, readmission/reinstatement, etc.
- Nursing Program Service Learning Form | This form should be completed to report volunteer hours.
- Letter of Recommendation Form | One request form is needed for each instructor.
- Change of Address Form | The department must be updated with any changes in your information. Please be sure to update Admissions & Records also, as our databases are separate.
- Student Portfolio Tracking Document | It is the responsibility of each nursing student to keep a hard copy or electronic copy of their graded written assignments listed.
HANDBOOKS
- Nursing Program Student Handbook | This resource is available to students who are currently enrolled in the RN Program at GCC.
- Nursing Program Faculty Handbook | This resource is available to faculty members who are currently teaching in the RN Program at GCC.
- Nursing Program Policy Handbook | This resource is available to students who are currently enrolled and faculty members who are currently teaching in the RN Program at GCC.
- Nursing Program Preceptorship Handbook | This resource is for students and faculty who are currently in Preceptorship under NS 214: Medical-Surgical Nursing IV.
